The South African Maritime Safety Authority (Samsa) has launched a preliminary enquiry into an incident involving the tug Umkhuseli that left three seafarers dead and two missing off Mossel Bay last week.

The incident occurred approximately 47 nautical miles south of Mossel Bay on Wednesday, when a crew member reportedly fell overboard while working on deck.

“In response, a Fast Rescue Boat (FRB) carrying four crew members was launched from the vessel to recover the individual. The FRB subsequently capsized, resulting in a total of five persons entering the water,” Samsa said.

The Maritime Rescue Co-ordination Centre (MRCC) Cape Town activated a coordinated search and rescue response, mobilising maritime and aviation resources.

Vessels in the vicinity were diverted to assist, with support from the National Sea Rescue Institute and SAPS water wing and helicopter resources.

Three bodies were recovered during the search, while two people remain missing.

“Following a comprehensive assessment of the search and rescue operation and consideration of all available information, the active search and rescue was concluded,” Samsa said.

The authority said its preliminary enquiry followed the conclusion of the active search. It extended its condolences to the families, loved ones, friends and colleagues of the deceased seafarers.

Meanwhile, Cosatu in the Western Cape has called for a thorough investigation to establish the cause of the incident and ensure accountability.

“We further call on employers to provide proper care and support to the families of the deceased and the missing,” the federation said.
